Dog License Application Form

The Township of Georgian Bay requires all dogs living within the municipality to be licensed with the Township. Animal Control By-law 2024-030 is the by-law that regulates variour matters realting to the keeping, licensing, and controlling of animls inthe Township of Georgian Bay.
 
In order to recieve your lifetime dog tags, all dog owners must complete and submit the following application form.

PLEASE NOTE: 

  • The fee is $50.00 per dog and is good for the lifetime of the dog.
  • No person shall keep no more than three (3) dogs at anyone time on one property.  This excludes the communities of Port Severn, Honey Harbour and MacTier, which only allow two (2) dogs to be kept on one property at any one time.

Prior to beginning application, you must prepare all information/documentation below:

  • Dog(s) name
  • Dog(s) age
  • Dog(s) sex
  • Dog(s) breed
  • Dog(s) colour
  • Dog(s) proof of rabies vacination

 

Prior to beginning application, I/WE acknowledge that I/WE have prepped all the required information/documentation mentioned above as I/WE will be required to input and upload all documentation prior to submitting application
 
Prior to beginning application, I/WE acknowledge that the fee is $50.00 per dog and is good for the lifetime of the dog.

Prior to beginning application, I/WE understand that I cannot save this application. The application must be submitted before the session ends. There is no save feature to resume an application at a later time.
